HOW DO I MAKE AN APPOINTMENT?

You will be contacted for your first appointment after your completed application has been reviewed.  Follow-up appointments will be made at time of service or if you have a change in condition and need to see the physician prior to your next appointment you can call the clinic and speak with the nurse.
